I understand that reasonable precautions will be taken to ensure the safety and well- being of my child during the event. I, also, understand by allowing my child to participate in MEGA Fun Days there is a risk of injury. I hereby release, discharge, and hold harmless, Centerville Community Church, its employees, volunteers and other representatives from any claims arising out of or relating to any physical injury that may result to the registering player while participating in MEGA Fun Days. In the event of an emergency, l authorize the staff/volunteers to seek medical treatment for my child if necessary. I also understand that I will be promptly notified in case of any emergency.
